White Hair Movement

February 2023

Background: In February 2023 winter had not yet left Wuhan. Two months earlier, three years of zero-COVID had ended almost overnight; a wave of infections swept the country, hospitals and crematoria were overwhelmed for a time, and many elderly people did not survive that winter. Those who did had barely caught their breath when they found the money in their medical insurance accounts had shrunk. From February 1, Wuhan's outpatient mutual-aid reform of employee medical insurance took effect, and the monthly sum paid into retirees' personal accounts was cut by more than half at a stroke. For pensioners living on a pension and relying on that account to buy medicine and see doctors, this was not an accounting exercise: it was roughly two hundred yuan a month of what they called life-saving money, gone. The news spread through retirees' chat groups, housing compounds and markets, and anger built up day by day. On February 8 and February 15, thousands of white-haired retirees took to the streets of Wuhan in what later came to be called the White Hair Movement. It was the first large street protest in China after the end of zero-COVID, and those who marched were not the young, but a generation of retired workers who had given their working lives to their work units and factories.

The old system: To understand the retirees' anger, one first has to understand the old arithmetic of employee medical insurance. China's basic medical insurance for urban employees was built around 1998, and the money sat in two pockets. One was the pooled fund, fed mainly by employer contributions, which covered hospital stays and major illness — a common pool for everyone. The other was the personal account: the employee's own contributions went in in full, and a share of the employer's contribution was also credited by age. It belonged to the individual, and it was what people swiped for minor outpatient visits and for medicine at pharmacies. Retirees paid no more contributions; their personal accounts were topped up monthly from the pooled fund. In Wuhan before the reform, the credit was a percentage of the individual's previous year's pension, rising with age, roughly between 4.8 and 5.1 percent. Over decades, the personal account had become, in elderly minds, unmistakably their own money: visible, tangible, saveable for a grandchild's illness. Anyone who touched it would have to answer to them first. The system's flaws were real enough: money slept idle in the accounts of the young and healthy while the seriously ill ran short, and cash locked up under individual names left the common pool strained. That was the argument officials repeated when they pushed the reform.

What the reform changed: The reform was not Wuhan's own idea. In April 2021, the General Office of the State Council issued national guidance on building an outpatient mutual-aid mechanism for employee medical insurance, requiring localities to bring outpatient costs into pooled coverage step by step while changing how personal accounts were credited. Wuhan's implementing document, the Implementation Rules for Outpatient Mutual Assistance Protection of Wuhan Basic Medical Insurance for Employees, took effect on February 1, 2023. It had two core provisions. First, for employees still working, the employer's entire medical insurance contribution would go into the pooled fund, with no share credited to personal accounts any longer; the personal account would keep only the employee's own contribution of 2 percent. Second, for retirees, the personal account would be credited a fixed sum from the pooled fund, no longer tied to the individual's own pension but calculated as 2.5 percent of the city's average basic pension of the previous year. Wuhan's medical security authorities did the sums themselves in a policy explainer: after the reform, retirees' personal accounts would fall by an average of about 1,992 yuan a year, and working employees' by about 1,373 yuan a year. By design, the money saved would all flow into the pooled fund, to pay for a new outpatient reimbursement benefit.

The numbers on a retiree's card: Percentages in a document become concrete money on a card. Two figures circulated in reporting for what Wuhan retirees had received each month before the reform: Reuters, citing Wuhan residents, put it at about 260 yuan a month, while Voice of America reported 286 yuan — the difference reflects that people of different ages and pension levels had never received the same amount in the first place. After the reform, everyone was levelled to 83 yuan a month, calculated as 2.5 percent of the average pension. A 60-year-old protester from Qingshan District laid out the arithmetic to VOA: more than 200 yuan less a month, more than 2,000 yuan less a year. To the well-off that is nothing; to a pensioner drawing two or three thousand yuan a month and taking blood-pressure and diabetes medicine year-round, it is the question of whether the medicine gets taken on time. A Wuhan resident quoted by Reuters put it more bluntly: the sum is very small, but to old people it is life-saving money, and when people are not prosperous every bit of it matters enormously. Another detail stung many people: by some reports, even the maximum funeral subsidy was sharply reduced in the same adjustment. The retirees' feeling could be summed up in one sentence: three years of epidemic control had emptied the insurance fund, and now their medicine money was being taken to plug the hole — without anyone asking them a single word first.

The design, and the official case for it: In the official design, the arithmetic was not supposed to be a simple subtraction. The Wuhan Municipal Medical Security Bureau explained repeatedly, in a statement on February 9, 2023 and in several policy notes that followed, that after the reform the pooled fund would be larger, its ability to pay stronger and its coverage more comprehensive. A new general outpatient benefit would bring clinic costs — previously borne almost entirely out of personal accounts — into reimbursement, at roughly 50 percent for working employees and about 60 percent for retirees, with higher rates steered toward primary-care clinics; arrangements were also made for pharmacy purchases and for chronic and special outpatient diseases. The bureau argued that those who gained most were precisely the frequently ill and the elderly, that every yuan removed from personal accounts had become reimbursement money in the pooled fund, and that overall benefits had not fallen but merely changed form. After the protests, the National Healthcare Security Administration said the same in public: less into personal accounts, more into the pool, outpatient coverage created where none had existed — money moved to where it cut deepest. None of this was without logic; outpatient risk-pooling is standard practice in many countries' systems. But the retirees asked a different set of questions. If this was a good thing, why was there no prior publication, no hearing, no transition period? And if the pool now held more money, why had no clear account ever been laid before the insured showing how much three years of epidemic control had spent from the fund, and how much remained?

Why the retirees rejected the arithmetic: The people who gathered had an arithmetic of their own. First, outpatient reimbursement came with a deductible threshold, a percentage and an annual cap. Someone who saw a doctor only a few times a year for minor ailments would never reach the threshold at which reimbursement began, while the personal-account cut arrived in full, every month. Second, the elderly's most routine expense was medicine bought at a pharmacy, previously a simple swipe of the personal account; after the reform, pharmacy reimbursement rules were stricter and designated outlets fewer, and many found that their regular medicines either could not be claimed or required a hospital prescription first — an exhausting round trip. Third, and most wounding, was trust. In retirees' eyes the personal account was money taken bit by bit from their wages when they were young and held under their own names; for the government to rewrite the crediting rules by decree felt no different from reaching into their pockets. In a widely shared recording of a phone call with the medical insurance authorities, one retiree said: this money was earned through our blood and sweat; I am complaining because I believe the government and the Communist Party can find a way forward. That sentence captures the character of the White Hair Movement: most of those on the street were not setting out to oppose anyone. They wanted an explanation, and they wanted back what they believed was theirs.

February 8: the first gathering: On Wednesday, February 8, 2023, in light rain, thousands of retirees began gathering from the morning onward in front of the Wuhan city government and around nearby Zhongshan Park — thousands, in the tally compiled from Central News Agency and other reporting. Most carried umbrellas; the crowd was orderly and peaceful, with no unified organizer and no printed placards. Many had simply heard the word passed through housing compounds and chat groups, ten telling ten, a hundred telling a hundred. They sang The Internationale together and shouted for the city government and the medical security bureau to come out and give an explanation, demanding that the pre-reform method of crediting personal accounts be restored. According to multiple media reports, city officials came out to talk with representatives of the protesters and record their demands. The retirees left a message on the spot: if there was no answer within a week, they would come back on February 15, and in larger numbers. That one-week pledge became the great open question over Wuhan's streets for the next seven days: officials studied their response, retired workers linked up in chat groups, and the meeting point passed from mouth to mouth shifted from the city government gate to Zhongshan Park.

The one-week pledge and the first official reply: On February 9, the day after the protesters dispersed, the Wuhan Municipal Medical Security Bureau issued an explanatory statement whose core message was that after the reform the pooled fund would be larger in scale, stronger in paying ability and more comprehensive in protection, that insured employees could now claim reimbursement for outpatient visits, and that the smaller personal accounts were a structural adjustment, not a cut in benefits. The statement did not mention the street gathering itself, and it promised no change to any rule. Over the following days the bureau published a run of policy Q&As answering, item by item, why personal accounts had shrunk, how outpatient reimbursement worked and what to do about pharmacy purchases, and local media carried worked examples purporting to show that sick retirees would actually spend less after the reform. National-level messaging fell into line; one official account run by the medical insurance system wrote that many elderly people had taken to the streets only because they had been bewitched by rumors. None of it calmed the doubts. What spread widest in retirees' chat groups was not the bureau's Q&A but photographs of handwritten household ledgers: pensioners posted their January and February credit screenshots side by side, the figure of 83 yuan repeated image after image. In this way, amid the official explanations, the appointment of February 15 drew closer day by day.

February 15: Zhongshan Park and Liberation Avenue: February 15, 2023 was the appointed day, and the Wuhan gathering was visibly larger than on February 8. From early morning the crowds converged on Zhongshan Park, which stands beside Liberation Avenue in one of the busiest districts of central Wuhan. Reporting by Reuters, AFP and The New York Times all confirms that the park entrance and surrounding streets filled with protesters who were overwhelmingly elderly, while a second group gathered not far away near Union Hospital. The Internationale was sung again, slogans such as give back our medical insurance money were chanted, and people held phones aloft filming throughout. The crowd estimates must be kept separate by source: Reuters and AFP reported in terms of hundreds to thousands on the scene, and the Times described hundreds on February 8 and a larger turnout on the 15th. The figures of tens or even hundreds of thousands circulating on social media were adopted by no outlet that verified from the scene, and this site records them as unverified. What is certain is that this was one of the largest street gatherings in a Chinese city since the White Paper Movement of November 2022 — and that its participants were almost uniformly retirees, something exceedingly rare in China's recent record of protest.

Handling at the scene: police, metro and shoving: Compared with the relative restraint of February 8, the handling on February 15 was a clear escalation. Video geolocated and verified by AFP showed security guards and police forming a human chain at the entrance to Zhongshan Park to stop more protesters entering; a clip verified by Reuters showed pushing and shoving between protesters and uniformed security personnel, with some people trying to climb over barriers and being pulled back. According to The New York Times and other outlets, metro trains for a time did not stop at the station nearest the park that day, in an apparent attempt to curb attendance — a detail confirmed by many who were present. The gathering lasted into the afternoon before the crowds gradually dispersed. As for how many people were taken away that day, claims range from single digits to dozens; all come from single channels or participants' relays, with no official notice and no second independent outlet verifying them, and this site marks all of them unverified. What can be confirmed as follow-up is this: Reuters reported that some Wuhan residents were afterward told not to hold illegal protests, gatherings or demonstrations in public spaces, and not to post protest content on social media. Radio Free Asia quoted a protester surnamed Gao as saying that after February 15 the authorities were calling people and detaining people one by one, and that he dared not say more for fear his phone was monitored. These individual accounts likewise cannot be independently verified, and this site records only that they exist.

Dalian the same day, and echoes elsewhere: February 15 was not Wuhan's day alone. That same morning, from around nine o'clock, hundreds of retirees in Dalian, Liaoning, gathered at People's Square, where the city government sits. AFP cited a witness confirming the gathering and geolocated video from it: the crowd chanted give back our medical insurance money and demanded the mayor come out, columns of police guarded the government buildings around the square, and the gathering dispersed by the afternoon. Reuters likewise confirmed a protest in Dalian that day, while noting that it could not immediately verify some of the Dalian images. Wuhan and Dalian are more than 1,200 kilometres apart and their pensioners were strangers to one another, yet their demand was identical word for word, because what had triggered both was the same outpatient mutual-aid reform being rolled out nationwide. Social media also carried videos and posts claiming echo gatherings by elderly people in places such as Anshan in Liaoning, but no wire service or major outlet verified them on the scene, and their times and places cannot be cross-checked; this site records them as unverified single-source claims. Reports of small-scale expressions of discontent by retirees in Guangzhou and elsewhere in early February are in the same position, lacking multi-source confirmation. The cross-city echo that belongs in the body of this archive is Wuhan and Dalian alone, both verified on the scene by international wire services.

What they demanded: Gathering the chants from both cities and the handwritten demand sheets that circulated, the White Hair Movement's demands were not complicated. First, restore the pre-reform method of crediting personal accounts — the demand that appeared in more chants than any other, which the retirees treated as their bottom line. Second, publish the medical insurance fund's accounts: exactly how much three years of epidemic control had spent from the fund, how much remained in the pooled fund now, and precisely where the money saved by the reform would flow — an accounting owed to the insured. Third, require that major changes of this kind solicit the insured's views in advance, rather than being imposed by a single document. Fourth, some chants on the scene also asked why the reform had come first for ordinary retired employees, and whether the benefits of civil servants and public-institution staff were being adjusted in step. Wuhan officials replied that civil servants' benefits were in fact being cut the most and that contrary claims were rumors, but they never published comparable figures, and the question mark has hung there ever since. The demands share one feature: not one of them reaches beyond medical insurance itself, and no chant called for overturning the heavens. The sentence the retirees repeated was give back our medical insurance money. That was both the movement's strength and the point at which the official narrative found it hardest to smear as foreign manipulation: those on the street were elderly people singing The Internationale and saying, over and over, that they believed in the Party and the government.

The official response, and what followed: The protests did not win a reversal of the reform. Through February, the Wuhan medical security bureau issued a string of statements and Q&As with an unchanging line: the direction of the reform would not change, every yuan removed from personal accounts had gone into the pool for outpatient reimbursement, retirees' outpatient reimbursement rate was higher than that of working employees, and overall benefits had risen rather than fallen. The National Healthcare Security Administration then voiced support for the outpatient mutual-aid reform, and Finance Minister Liu Kun, in an article published in mid-February, vowed to deepen reform of medical insurance payment methods — an article that made no mention of the protests themselves. From the centre to the localities, in other words, the answer to the protests was explanation and publicity, not concession. The reform train rolled on as scheduled: within 2023, the great majority of pooling regions nationwide completed their outpatient mutual-aid rollouts, and the new personal-account rules were fully in force. In the year or two that followed, some cities, under public pressure and in light of operating data, made small adjustments to outpatient pooling details — lowering or abolishing the outpatient deductible at primary-care clinics, raising annual reimbursement caps, bringing more designated pharmacies into outpatient settlement — adjustments documented in local medical security bureaus' public files. But no city restored the pre-reform method of crediting personal accounts. The White Hair Movement's central demand was, in the end, not met.

The white hair online: censorship and guidance: The street gatherings lasted two days, but online the White Hair Movement left almost no public trace. On February 15 itself, videos from Wuhan and Dalian first spread in volume across WeChat, Weibo and short-video platforms, and were then deleted in batches, with many reposting accounts restricted. Searching combinations such as Wuhan and medical insurance protest returned only the bureau's policy publicity and local media's positive worked examples. Official opinion guidance unfolded in parallel: the medical insurance system's official account wrote that the elderly had been misled by rumors, local media published individual stories of cheaper care after the reform, and commentators aimed at the hype of foreign media. The handling followed the White Paper Movement's pattern exactly: delete the videos first, fix the characterization second, and finally leave search results containing nothing but the official version. And as with the White Paper Movement, deletion failed to delete memory. Videos of February 15 were mirrored by internet users to overseas platforms and became the main source material for international reporting; the very name White Hair Movement first circulated beyond the firewall before becoming the standard name for this episode. Anyone inside the wall who wants another look at Zhongshan Park that day has to go outside the wall to find it.

Two months after the White Paper Movement: The coordinate the White Hair Movement cannot escape is its timing relative to the White Paper Movement. In late November 2022, the largely youthful White Paper Movement erupted in cities across China, chanting directly against zero-COVID; a dozen or so days later the authorities announced that zero-COVID was over. Between the end of zero-COVID and the white-haired retirees taking to the streets lay only about two months. In a single winter, two generations stood up in China's streets in succession: first the university students and young citizens holding blank sheets of paper, then the retired workers singing The Internationale. The two generations' demands look different — one heated, one mundane — but their roots are joined. The young were opposing control itself; the old were contesting the bill that control had left behind. The enormous cost of three years of epidemic control had ultimately to be paid by someone, and when the bill was handed to the elderly in the form of a medical insurance reform, the second wave of protest arrived. Freedom House's China Dissent Monitor, commenting on the White Hair Movement, noted that isolated protests are common in China, but this topic was notable. That is why this site files the White Hair Movement immediately after the White Paper Movement: it is the White Paper Movement's aftershock, and the other half of the puzzle for understanding public feeling in post-zero-COVID China — the first half written on the young people's white paper, the second on the old people's white hair.

International response: The White Hair Movement entered the top tier of international news on the day it happened. Reuters filed on-the-scene reporting from both Wuhan and Dalian on February 15, with verified video from Zhongshan Park. AFP the same day confirmed both gatherings — in front of the Wuhan city government and at Dalian's People's Square — and geolocated the imagery. The New York Times, reporting on February 15–16, set the two days of gatherings against the larger background of local finances hollowed out by three years of epidemic control. An Associated Press dispatch, carried by many outlets, called the events a rare public protest in China in recent months, and National Public Radio's February 16 report stressed that the unrest had been triggered by the largest medical insurance reform in more than two decades. The reporting shared three common threads. All of it noticed the protesters' age structure — retired elderly people on the street are exceedingly rare in China. All of it placed the White Hair Movement alongside the White Paper Movement of two months earlier, as a continuation of post-zero-COVID discontent. And all of it pointed to the fiscal thread running underneath: the huge cost of zero-COVID, shrinking land-sale revenues and accelerating ageing were squeezing local government ledgers at the same time. The coverage did not change the reform's course, but it ensured that a complete record of the episode survives outside the firewall — the principal source base for this archive entry.

Crowd numbers: the February 8 Wuhan gathering numbered in the thousands, supported by the Central News Agency and other reporting; for February 15 in Wuhan and Dalian, wire-service figures range from hundreds to thousands, while social-media claims of tens or hundreds of thousands are marked unverified. Credit figures: the two pre-reform monthly figures, 260 yuan and 286 yuan, coexist in the record, from Reuters' resident accounts and Voice of America's reporting respectively; the difference arises because credits were previously tiered by age and pension level, and this site records both in parallel rather than merging them. Detentions: every specific figure for people taken away on the day, or called in or detained one by one afterward, derives from a single channel or a participant's relay, with no official notice and no second source, and is marked unverified. Echo gatherings: claims of gatherings in Anshan, Guangzhou and elsewhere rest on single-source social-media material only, and are marked unverified. The reported official promise of an answer within a week derives from the shared account of protesters and media at the scene on February 8; the authorities never confirmed such a deadline in any document, and this site records it as an on-the-scene account. Latest status: as of this site's last check in 2026, Wuhan's outpatient mutual-aid reform has never been revoked, the fixed personal-account credit of around 83 yuan remains in force, and there is no public record of the subsequent situation of White Hair Movement participants, nor any public reporting of a comparable medical-insurance protest in either city since. Where nothing can be found, this site writes, plainly, that there is no public record.
